Privacy Policy

Laurentians Eye Institute ("LEI") attaches great importance to the confidentiality and security of the personal information provided by its clients and employees. This policy attests to this concern and our commitment to protect such information. The related procedures specify how it shall be applied. The two major issues treated are the way companies gather, hold, use, communicate and protect personal information and the right of individuals to refer to such information and to have it amended if necessary. The policy applies to the personal information in all its forms – verbal, electronic, written or other – that a person gathers, holds, uses or communicates to third parties in connection with a company's business activities. A person shall be informed of and consent to any gathering, use or communication of personal information about him or her, unless it is not appropriate to do so. Personal information gathered from third parties such as an authorized agent, or professional or personal references, shall be done with the permission of the individual concerned and the source of the information shall be recorded in his or her file. The only time in which LEI shall gather or communicate personal information without the consent of the person concerned shall be under circumstances required by law.



The LEI may collect your personal information from third parties such as your doctor, optometrist, hospital or your authorized representative. The LEI may disclose your personal information to these third parties, the governments of Canada, provincial or municipal agencies of those governments in the preparation of your intervention or consultation to the LEI. 


Health insurance number

The LEI is required by law to use your health insurance number when communicating with third parties mentioned above.


Collection, hold, use and disclosure of personal information.

The LEI will only collect personal information necessary to complete the necessary documentation, determine eligibility and / or to comply with regulations, policies, laws and legislation.

The LEI retain personal information only when necessary to meet the purposes stated at the time it was collected. If it is determined that an individual's personal information is no longer needed, the LEI will take steps to destroy or delete this information or arrange so that the information can not be collected or used by any unauthorized party.

When you enter the website, you move from page to page, read pages or download content to your computer, the following information may be used:

The LEI can use this information to determine what types of information are most useful to improve its website meet your specific needs, provide better service to patients and the general marketing purposes. Complaints and appeals

The LEI handle complaints and disputes arising from the collection, use and disclosure of personal information. To deal with the complaint adequately, all details will be recorded and a file will be created. All persons concerned will be notified that an investigation has been launched relatively to the complaint. If you decide to bring an action, all parties involved will be informed of the outcome

The measures described in this privacy policy are intended to reassure patients that the staff treats their information importantly.
